Primary Home Care
Purpose
Primary Home Care is a program for Medicaid beneficiaries whose chronic health
problems cause them to be functionally limited with daily living activities.
This program is federally funded and administered under contract with the Texas
Department of Aging and Disability Services. Primary home care provides non-nursing,
medically prescribed and supervised personal care and house-keeping services.
Available Services
- Personal
care (dressing and bathing).
- Toileting and transfer.
- Household tasks that relate to the patient’s comfort
and personal environment (laundry, cleaning).
- Meal preparation for the patient.
- Escort service for the patient requiring
a physician appointment.
Community Based Alternative
The Community Based Alternative
(CBA) program provides home and community-based services to persons 21 and
older as cost-effective alternatives for those who qualify for nursing facility
care. The program goal is to provide patients with meaningful choices about
long-term care by offering services that support the patient and avoid premature
nursing home placement. CBA also provides current nursing home residents an
opportunity to return to the community.
Criteria
- Patient must be a Medicaid beneficiary.
- Patient must meet financial and
physical need criteria as determined by the Texas Department of Aging and
Disability Services.
